Lease/Hire/Rental Company and Customer Appeal Process

This form should be used by lease/hire/rental companies and customers. Customers can only appeal if their lease/hire/rental company has provided authorisation for you to do so. Please see this link. If you appeal and an authorisation has not been provided, your appeal will not be accepted and you must appeal via your lease/hire/rental company.
